A Claude Code mod in avishalom/session-note

Tell your Claude Code sessions apart. /note pins a context comment to a session and /note -color gives it a color: the comment shows in a colored band above the prompt, and in the desktop Code tab the session's sidebar title gets a matching square emoji (🟥🟧🟨🟩🟦🟪) and the comment.
